Samenvatting
Limbische encefalitis (LE) is een relatief zeldzame aandoening van het limbische systeem, die tot op heden weinig bekend is en daarom vaak laat wordt gediagnosticeerd. Het gaat hier om een ontstekingsproces dat vaak geen micro-organisme als verwekker heeft, maar het gevolg is van een auto-immuunreactie. In een deel van deze gevallen is er sprake van een paraneoplastisch proces waarbij een tumor de auto-immuunreactie in gang zet, maar in een ander deel van de gevallen blijft onduidelijk hoe de auto-immuunreactie is ontstaan. Bij ongeveer 50% van de patiënten met een auto-immuun-gemedieerde LE kan er een specifiek antilichaam worden vastgesteld.
Het klinische beeld en de prognose van LE kunnen sterk verschillen, afhankelijk van het veroorzakende antilichaam. De klassieke presentatie bestaat uit geheugenstoornissen, verwardheid en epileptische aanvallen, maar daarnaast kunnen vele andere neurologische en psychiatrische symptomen optreden. Daarom wordt een LE gemakkelijk aangezien voor een neurologische of psychiatrische aandoening die veel gangbaarder is. De behandeling is in een deel van de gevallen zeer succesvol. Van cruciaal belang voor de uiteindelijke prognose zijn een snelle diagnose en een spoedige behandeling. Omdat een LE sterk kan lijken op andere psychiatrische en neurologische aandoeningen, is het van belang dat zowel neurologen als psychiaters het beeld kennen en weet hebben van de behandelmogelijkheden. In dit artikel worden twee gevalsbeschrijvingen gepresenteerd die illustreren hoe verschillend een LE kan verlopen. Daarnaast wordt er een overzicht gegeven van de verschillende vormen van LE.
