When you use Aflac SmartClaim^® to submit accident, cancer, hospitalization or illness claims online you’ll get paid fast.

Register or log in:

1. Go to aflac.com/myaflac or download the MyAflac Mobile app from the App Store or Google Play Store.

Enroll in claims direct deposit:

1. Direct Deposit allows you to get paid fast.
2. Click Direct Deposit and follow the instructions for registration.
3. Please allow one business day for Direct Deposit enrollment to take effect.

File your claim with Aflac SmartClaim®:

1. Access Aflac SmartClaim from MyAflac or the MyAflac Mobile app.
2. Aflac SmartClaim guides you every step of the way.
3. Upload required documents.¹

Please use the claim appeal form to organize your request. Please be sure to explain why you disagree with Aflac's decision, and include any additional supporting documentation.

You have the right to appeal a decision up to a maximum of three times per claim.
All appeals must be submitted within 180 days of the original claim decision.
