Support Blue Gives 

Choices to Patients 

Ginny’s dogs Murphy and Max are a great comfort to her. 

Former elementary school teacher Ginny Lietz knows what it is like to feel overwhelmed 

with stress. In 1990, the East Aurora resident was diagnosed with a brain 

tumor and had surgery to remove it. 

“I was really on the road to recovery. I lived for 15 years basically in remission,” 

recalls Ginny. “In 2004 around Christmastime, I suddenly started to fall for no 

apparent reason…it was a recurrence of the tumor.” 

Later, during a physical therapy session, Ginny didn’t feel well. “I began to 

experience what was the first of many seizures…as a result of multi-seizures and 

a stroke, I became a tri-plegic.” 

While she has a close circle of friends, Ginny felt helpless. That’s when she learned 

about Support Blue, a specialized home care program that coordinates services for 

people with serious illness and their loved ones. The special BlueCross BlueShield 

and Community Blue program is offered through Life Transitions Center. 

“Jeri, my counselor, helped me to see what was available to me… I think the 

mere fact that you’re given choices is a rejuvenating experience. We talked about 

finances, we talked about nursing home care, home care and the kind of services 

that are available. We talked about quality of life issues… She listened with a 

non-prejudiced point of view. I was thrilled to escape. I thought I didn’t have many 

choices in life. Jeri showed me that I had a whole lot more than I realized…it’s 

wonderful to have someone say ‘Look at what you could do, what you can do.’” 

According to Ginny, Support Blue not only helped her physically, but emotionally. 

“Because you’re ill and you have a long term illness, you need not always dwell on 

the illness itself,” she smiles. “There are many things you can do in your life. It’s 

important to be positive. Thank you to Life Transitions Center for the positive 

attitude adjustment and growth.”

Hospice Benefit would mean drastic changes for our Hospice program and 

could limit our ability to deliver quality care when it is needed the most. 

As many of you know, Hospice is a Medicare benefit. The Medicare 

Hospice Benefit was established in 1982 to ensure that all Medicare 

beneficiaries can access high-quality end-of-life care. More than 80% of 

hospice patients are Medicare beneficiaries.* 

We have determined that the Administration’s proposed cuts will result 

in a $1.2 million loss to Hospice Buffalo over the next 3 years. We are very 

concerned that these cuts will have a direct impact on care at the bedside 

and need your help to tell your congressional representatives to oppose any 

cut to Hospice reimbursement. 

We have enclosed a sample letter (see page 4 of this issue) that can be 

used as a guide to write or to call your representative. The addresses, email 

and phone numbers of Western New York State congress members and the 

sample letter are also available on our website at www.hospicebuffalo.com 

under “Newsroom” located at the bottom of the Hospice home page. 

Thank you for considering our appeal to contact your federal representative. 

Your voice, speaking for the patients and families we serve, needs to be

Calling Hospice was the Best Decision 

“My mom was always a very strong person,” recalled Irma Martinez, a 

mother of two children and her mom’s caregiver in their Buffalo home for 

the last two and a half years. “She worked all her life. She was never sick 

until she got brain cancer. She was the provider for me and my brother.” 

“We started Hospice in December 2007, because my mom’s cancer had 

gone from being in remission and had come back aggressively. There were 

no other treatments for us to do for her. With the oncologist, we decided to 

bring in Hospice. That was the best decision I made as a daughter because 

as soon as I got a hold of Hospice, they immediately called me and the next 

day they came here. They provided us with home delivery for prescriptions 

so that I didn’t have to leave the house. They set me up with a chaplain, 

Fr. Bob. They gave me a phone number to reach them 24-hours a day, 

seven days a week so if I have any questions, I can call Hospice.”

Leslie Greene, RN, from Hospice visited the 

Hernandez home once a week to check Irma’s 

vital signs, weight, and medicine. “They were 

also able to give me a home health aide who 

comes in 2 hours a day to help me with my 

mom’s bathing, housework, and a little washing. 

She also gives my mom a snack, if needed, 

and talks with her,” smiled Irma as her mom 

rested comfortably in her bed. 

With the help of the Hospice team, Irma’s mom 

was able to remain at home with her family. 

“Hospice is not just there to come in and see how things are going,” 

explained Irma. “They also help you with other services such as contacting 

Meals on Wheels or talking with a social worker. This past weekend, we 

had a family reunion and Hospice was able to provide me with a volunteer 

beautician to do my mom’s hair and arranged a nice photographer to take 

family pictures. Hospice goes above and beyond to make sure that your 

family needs are taken care of.” 

While Irma was not able to spend a great deal of time with her mom during 

childhood, she has found that her mom’s illness has enabled the two of them 

to be closer. “Now that she’s sick, it’s brought us together, because growing 

up she worked very long hours….I’ve been able to find out things about 

her that I never knew before… She’s made me the person that I am today. 

I’m a strong individual because of her. I have very high values and I set 

high expectations… I’m able to be a responsible adult and have dealt with 

a lot of obstacles in my own life. I’ve been a cancer survivor for 12 years. 

My mom taught me not to give up and I know that she will keep fighting 

this disease until God tells her it’s time to go. . .If anyone has someone 

with a terminal illness, I would recommend Hospice because Hospice has 

been excellent for my family.”

day by Hospice Buffalo, a not-for-profit agency celebrating its 30th year of 

service to our community. As you may know, Hospice care is provided by a 

team of professionals including physicians, nurses, social workers and spiritual 

caregivers who work together to bring quality of life to individuals facing a 

life limiting illness. Hospice also provides important end-of-life care education 

and support to our community-at-large, through programs offered by the 

Center for Excellence in End-of-Life Education, Research & Practice, and 

CMI Education Institute which trains, annually, more than 20,000 health 

care and allied professionals nationwide. Together, Hospice Buffalo programs 

impact thousands of your constituents each year. 

On behalf of Hospice Buffalo in Erie County, New York, I urge you to protect 

the Medicare hospice reimbursement rates. President Bush’s Administration’s 

budget proposal contains a number of proposed reductions to the reimbursement 

structure for hospice programs. Any cuts to the Medicare Hospice 

Benefit would mean drastic changes for our community’s hospice program 

and could limit access to Hospice services and its ability to deliver quality 

care when it’s needed the most. Proposed cuts will result in a $1.2 million 

reduction in revenue for Hospice Buffalo over the next 3 years. The 

Administration’s proposal comes on the heels of a recent research study 

conducted by Duke University which found that hospice reduced Medicare 

costs by an average of $2,309 per hospice patient. The proposed action is, 

thus, careless and contrary to the goal of saving Medicare dollars. 

While you and your colleagues continue work on the federal budget 

proposal, I ask your continued support for Hospice Buffalo as it provides this 

most necessary service to your constituents facing a life-limiting illness and 

their families, by opposing any cuts to hospice reimbursements. 

I respectfully ask you to reject any proposal that would impose cuts to the 

Medicare Hospice reimbursement rates.

A PILOT PROGRAM OF LIFE TRANSITIONS CENTER 

AND INDEPENDENT HEALTH 

Home Connections Will Help Frail 

Elderly Remain in Their Homes 

A new program created by Life Transitions Center and Independent 

Health will offer unique medical and non-medical support to enable more 

frail elders to stay in the safety of their own homes. 

“Erie County has one of the oldest populations in New York State,” 

noted Sara Montz, LCSW-R, director of Life Transitions Center. “As more 

and more baby boomers achieve ‘senior citizen’ status, increased services 

will be needed to keep these folks in their homes. Home Connections 

Home Care Demonstration Program will use the proven home-based case 

management of Life Transitions Center and existing community resources 

in partnership to enrich care for home-bound frail elderly.” 

Home Connections is expected to serve approximately 150 seniors who 

are frail and/or suffering from chronic disease which places them at risk for 

hospitalization or institutionalization in a long term care facility. For example, 

Independent Health members with recent hospitalizations who are at risk 

for re-hospitalization due to illness, safety or social factors will be offered 

Home Connections services. It is anticipated that clients with cancer, 

congestive heart failure, pulmonary disease, and chronic illness with 

multiple conditions, will benefit from a full range of palliative care services 

and support services to enable them to stay at home - safe, healthy and 

socially engaged.

A Call Center located within Life Transitions 

Center, and staffed by case managers, will 

be the hub from which all services will be 

coordinated. This Call Center will use stateof-the 

art technology to provide cost effective 

care management. Services provided by Home 

Connections will include a case manager to 

provide telemanagement and in-home care 

management planning; 24-hour on-call nurse 

and in-home assessments; 24-hour on-call 

home health aides; in-home palliative care 

physician assessment and visits; trained 

volunteers for telephone assurance, light housekeeping, 

meal preparation, shopping and pet care; connections to relatives 

and friends; emergency response team for ambulance, fire department, 

and utilities; transportation; lifeline; and outside services for legal, 

banking and home repair. 

While there are currently a variety of agencies that provide home-based 

support services, most costs for these services are not covered by Medicare 

or private insurance, especially if an older person has had a recent hospitalization. 

This places frail seniors who are on a fixed income at risk for 

avoiding outside help in their home. Home Connections partnership with 

Independent Health will help to address the need for quality, cost-effective 

support services to enable seniors to remain in their homes for as long 

as possible. 

Home Connections Palliative Care 

Management will directly provide or link all the social, 

health care, and quality of life issues necessary to make 

time at home the best option for Independent Health 

clients with serious illness. 

“The new Home Connections Palliative Care Management program 

will offer services that no other program does; combining health care 

service delivery with care management, volunteer visits, transportation 

and tele-management,” stated William E. Finn, president & CEO of 

The Center for Hospice & Palliative Care, the parent company of Life 

Transitions Center, Inc. 

After the pilot has proven successful, the program will be expanded to 

encompass all persons in Erie County who are at risk of hospitalization 

or institutionalization.

The Life Transitions Center and 

Essential Care for Children will be 

offering the 5th Annual Camp Blue 

Skies program, an overnight camp 

for any youth in Western New York, 

ages 7-16, who has experienced the 

death of a loved one three months 

to three years prior to the beginning 

of camp. Since Camp Blue Skies 

began in 2004, more than 145 youth 

have participated in the camp 

program. Camp Blue Skies offers 

therapeutic and recreational activities 

that allow children and teens to 

interact in age appropriate groups 

with professional grief counselors. 

Through small group activities and 

discussion, campers have the opportunity 

to share feelings, memories, 

and concerns about their loss. 

Campers connect with others who 

have experienced a similar loss and 

learn healthy ways to cope. 

Volunteers are needed as Camp 

Mentors to run activities, supervise 

campers and bunk in the cabins! 

It promises to be a meaningful 

experience for everyone involved. 

35- 40 volunteers are needed to help 

the weekend run smoothly. We are 

always looking for male and female 

volunteers, over the age of 18. We 

have a particular need to recruit 

male volunteers. Last year 29 of our 

46 campers were boys. It is critical 

to the success of the program to 

have positive male role models to 

work with the boys.

St. John Baptist Hospice 

Buffalo House Wish List 

St. John Baptist Church and Hospice Buffalo are pleased to announce that 

the St. John Baptist/Hospice Buffalo House, currently under construction 

on Maple Street in downtown Buffalo, will open in June of this year. This 

unique project is a collaborative effort between the church and Hospice. 

The St. John Baptist/Hospice Buffalo House will be the nation’s first 

residence for terminally ill patients developed jointly by a hospice program 

and a spiritual organization. This national model will provide hospice access 

and outreach services to residents of Buffalo’s East Side and throughout the 

broader community. 

In preparation for the opening residents, their family members, staff and 

volunteers have requested the following items. If you wish to donate any 

one of these items listed, please contact the Hospice Foundation office at 

716-686-8090 BEFORE PURCHASE to determine if the item is still needed.

The 13th Annual Artvoice Mardi Gras was held on February 5th in all 

three of Buffalo’s Entertainment Districts – Chippewa, Allentown and 

Elmwood. It began with a motorcade parade of over 30 floats through the area 

and ended with over 45 clubs participating in a New Orleans style celebration 

for everyone. Over $22,000 was raised for Hospice Buffalo. Thank you to 

Jamie Moses, Deborah Ellis, Rose Mattrey and the staff from ARTVOICE 

for coordinating this great event and to all the sponsors that helped make the 

night possible: Southern Comfort, Heineken, 103.3 The EDGE, Mix 104.1 

and 97 Rock. We would also like to thank all of the Hospice volunteers who 

volunteered their time that evening.

Spring Appeal 

Many of our donors and friends were recently sent a letter from Mary 

Ann Schuldes, a current Hospice Buffalo patient. Mary Ann suffers from 

ALS, Lou Gehrig’s disease, and receives hospice services in her home. The 

letter describes her gratitude for the care she is currently receiving. With 

Hospice services she is able to have quality time at home with her husband. 

With Mother’s Day, Father’s Day and Memorial Day falling within a 

few weeks of each other, these occasions offer the perfect time to respond 

generously with a gift to honor a loved one’s memory. Families like the

The 22nd Annual Hospice Spring Bouquet Sale 

was a terrific success. We are pleased to report that 

26,000 spring bouquets and 3,000 tribute bouquets 

were sold raising $254,000. All money raised benefits 

the Hospice Foundation supporting Hospice Buffalo, 

Hospice Buffalo House, Life Transitions Center, Inc.

Mission Statement: 

The Center for Hospice & Palliative Care serves individuals 

with serious illness, and their families and caregivers through 

a continuum of care spanning the course of illness and 

bereavement. The Center’s specially-trained professionals, 

support staff and volunteers assist individuals to make choices 

about their care, support those who are grieving and educate 

the professional and lay communities about end-of-life issues. 

How To Access Our Services 

Affiliates of The Center for Hospice & Palliative Care are governed 

by the State of New York and most services are covered by a 

majority of insurance companies, including Medicare/Medicaid. 

Referring to the The Center for Hospice & Palliative Care is 

simple and convenient. Just call (716) 686-8000. An intake specialist 

will work with the patient, the family, and attending physician 

to initiate services and establish an individual plan of care. 

The Center for Hospice & Palliative Care consists of the 

following agencies: 

a centrally coordinated program of end-of-life 

education, research and practice. 

provides national education and training of 

bereavement and health care professionals. 

Includes the American Academy of Bereavement. 

provides medical care, as well as emotional and 

spiritual support to patients with advanced illness 

and their families. 

A 10-bed residence offering comfort and security 

for hospice patients in the final months or weeks 

of illness. 

An 8-bed residence in Buffalo offering comfort and 

security for hospice patients in the final months or 

weeks of illness. Opening June 2008. 

provides counseling and support groups for children, 

adults and families concerned with grief, death 

and coping with a serious illness. 

accepts, administers and invests all charitable funds 

on behalf of The Center for Hospice & Palliative 

Care and its affiliate agencies. All gifts are tax 

deductible as provided by law.
