Support Blue Gives<br />

Choices to Patients<br />

Ginny’s dogs Murphy and Max are a great comfort to her.<br />

Former elementary school teacher Ginny Lietz knows what it is like to feel overwhelmed<br />

with stress. In 1990, the East Aurora resident was diagnosed with a brain<br />

tumor and had surgery to remove it.<br />

“I was really on the road to recovery. I lived for 15 years basically in remission,”<br />

recalls Ginny. “In 2004 around Christmastime, I suddenly started to fall for no<br />

apparent reason…it was a recurrence of the tumor.”<br />

Later, during a physical therapy session, Ginny didn’t feel well. “I began to<br />

experience what was the first of many seizures…as a result of multi-seizures and<br />

a stroke, I became a tri-plegic.”<br />

While she has a close circle of friends, Ginny felt helpless. That’s when she learned<br />

about Support Blue, a specialized home care program that coordinates services for<br />

people with serious illness and their loved ones. The special BlueCross BlueShield<br />

and Community Blue program is offered through Life Transitions Center.<br />

“Jeri, my counselor, helped me to see what was available to me… I think the<br />

mere fact that you’re given choices is a rejuvenating experience. We talked about<br />

finances, we talked about nursing home care, home care and the kind of services<br />

that are available. We talked about quality of life issues… She listened with a<br />

non-prejudiced point of view. I was thrilled to escape. I thought I didn’t have many<br />

choices in life. Jeri showed me that I had a whole lot more than I realized…it’s<br />

wonderful to have someone say ‘Look at what you could do, what you can do.’”<br />

According to Ginny, Support Blue not only helped her physically, but emotionally.<br />

“Because you’re ill and you have a long term illness, you need not always dwell on<br />

the illness itself,” she smiles. “There are many things you can do in your life. It’s<br />

important to be positive. Thank you to Life Transitions Center for the positive<br />

attitude adjustment and growth.”<br />

<strong>Hospice</strong> Benefit would mean drastic changes for our <strong>Hospice</strong> program and<br />

could limit our ability to deliver quality care when it is needed the most.<br />

As many of you know, <strong>Hospice</strong> is a Medicare benefit. The Medicare<br />

<strong>Hospice</strong> Benefit was established in 1982 to ensure that all Medicare<br />

beneficiaries can access high-quality end-of-life care. More than 80% of<br />

hospice patients are Medicare beneficiaries.*<br />

We have determined that the Administration’s proposed cuts will result<br />

in a $1.2 million loss to <strong>Hospice</strong> <strong>Buffalo</strong> over the next 3 years. We are very<br />

concerned that these cuts will have a direct impact on care at the bedside<br />

and need your help to tell your congressional representatives to oppose any<br />

cut to <strong>Hospice</strong> reimbursement.<br />

We have enclosed a sample letter (see page 4 of this issue) that can be<br />

used as a guide to write or to call your representative. The addresses, email<br />

and phone numbers of Western New York State congress members and the<br />

sample letter are also available on our website at www.hospicebuffalo.com<br />

under “Newsroom” located at the bottom of the <strong>Hospice</strong> home page.<br />

Thank you for considering our appeal to contact your federal representative.<br />

Calling <strong>Hospice</strong> was the Best Decision<br />

“My mom was always a very strong person,” recalled Irma Martinez, a<br />

mother of two children and her mom’s caregiver in their <strong>Buffalo</strong> home for<br />

the last two and a half years. “She worked all her life. She was never sick<br />

until she got brain cancer. She was the provider for me and my brother.”<br />

“We started <strong>Hospice</strong> in December 2007, because my mom’s cancer had<br />

gone from being in remission and had come back aggressively. There were<br />

no other treatments for us to do for her. With the oncologist, we decided to<br />

bring in <strong>Hospice</strong>. That was the best decision I made as a daughter because<br />

as soon as I got a hold of <strong>Hospice</strong>, they immediately called me and the next<br />

day they came here. They provided us with home delivery for prescriptions<br />

so that I didn’t have to leave the house. They set me up with a chaplain,<br />

Fr. Bob. They gave me a phone number to reach them 24-hours a day,<br />

seven days a week so if I have any questions, I can call <strong>Hospice</strong>.”<br />

Leslie Greene, RN, from <strong>Hospice</strong> visited the<br />

Hernandez home once a week to check Irma’s<br />

vital signs, weight, and medicine. “They were<br />

also able to give me a home health aide who<br />

comes in 2 hours a day to help me with my<br />

mom’s bathing, housework, and a little washing.<br />

She also gives my mom a snack, if needed,<br />

and talks with her,” smiled Irma as her mom<br />

rested comfortably in her bed.<br />

With the help of the <strong>Hospice</strong> team, Irma’s mom<br />

was able to remain at home with her family.<br />

“<strong>Hospice</strong> is not just there to come in and see how things are going,”<br />

explained Irma. “They also help you with other services such as contacting<br />

Meals on Wheels or talking with a social worker. This past weekend, we<br />

had a family reunion and <strong>Hospice</strong> was able to provide me with a volunteer<br />

beautician to do my mom’s hair and arranged a nice photographer to take<br />

family pictures. <strong>Hospice</strong> goes above and beyond to make sure that your<br />

family needs are taken care of.”<br />

While Irma was not able to spend a great deal of time with her mom during<br />

childhood, she has found that her mom’s illness has enabled the two of them<br />

to be closer. “Now that she’s sick, it’s brought us together, because growing<br />

up she worked very long hours….I’ve been able to find out things about<br />

her that I never knew before… She’s made me the person that I am today.<br />

I’m a strong individual because of her. I have very high values and I set<br />

high expectations… I’m able to be a responsible adult and have dealt with<br />

a lot of obstacles in my own life. I’ve been a cancer survivor for 12 years.<br />

My mom taught me not to give up and I know that she will keep fighting<br />

this disease until God tells her it’s time to go. . .If anyone has someone<br />

with a terminal illness, I would recommend <strong>Hospice</strong> because <strong>Hospice</strong> has<br />

been excellent for my family.”<br />

day by <strong>Hospice</strong> <strong>Buffalo</strong>, a not-for-profit agency celebrating its 30th year of<br />

service to our community. As you may know, <strong>Hospice</strong> care is provided by a<br />

team of professionals including physicians, nurses, social workers and spiritual<br />

caregivers who work together to bring quality of life to individuals facing a<br />

life limiting illness. <strong>Hospice</strong> also provides important end-of-life care education<br />

and support to our community-at-large, through programs offered by the<br />

Center for Excellence in End-of-Life Education, Research & Practice, and<br />

CMI Education Institute which trains, annually, more than 20,000 health<br />

care and allied professionals nationwide. Together, <strong>Hospice</strong> <strong>Buffalo</strong> programs<br />

impact thousands of your constituents each year.<br />

On behalf of <strong>Hospice</strong> <strong>Buffalo</strong> in Erie County, New York, I urge you to protect<br />

the Medicare hospice reimbursement rates. President Bush’s Administration’s<br />

budget proposal contains a number of proposed reductions to the reimbursement<br />

structure for hospice programs. Any cuts to the Medicare <strong>Hospice</strong><br />

Benefit would mean drastic changes for our community’s hospice program<br />

and could limit access to <strong>Hospice</strong> services and its ability to deliver quality<br />

care when it’s needed the most. Proposed cuts will result in a $1.2 million<br />

reduction in revenue for <strong>Hospice</strong> <strong>Buffalo</strong> over the next 3 years. The<br />

Administration’s proposal comes on the heels of a recent research study<br />

conducted by Duke University which found that hospice reduced Medicare<br />

costs by an average of $2,309 per hospice patient. The proposed action is,<br />

thus, careless and contrary to the goal of saving Medicare dollars.<br />

While you and your colleagues continue work on the federal budget<br />

proposal, I ask your continued support for <strong>Hospice</strong> <strong>Buffalo</strong> as it provides this<br />

most necessary service to your constituents facing a life-limiting illness and<br />

their families, by opposing any cuts to hospice reimbursements.<br />

I respectfully ask you to reject any proposal that would impose cuts to the<br />

Medicare <strong>Hospice</strong> reimbursement rates.<br />

A PILOT PROGRAM OF LIFE TRANSITIONS CENTER<br />

AND INDEPENDENT HEALTH<br />

Home Connections Will Help Frail<br />

Elderly Remain in Their Homes<br />

A new program created by Life Transitions Center and Independent<br />

Health will offer unique medical and non-medical support to enable more<br />

frail elders to stay in the safety of their own homes.<br />

“Erie County has one of the oldest populations in New York State,”<br />

noted Sara Montz, LCSW-R, director of Life Transitions Center. “As more<br />

and more baby boomers achieve ‘senior citizen’ status, increased services<br />

will be needed to keep these folks in their homes. Home Connections<br />

Home Care Demonstration Program will use the proven home-based case<br />

management of Life Transitions Center and existing community resources<br />

in partnership to enrich care for home-bound frail elderly.”<br />

Home Connections is expected to serve approximately 150 seniors who<br />

are frail and/or suffering from chronic disease which places them at risk for<br />

hospitalization or institutionalization in a long term care facility. For example,<br />

Independent Health members with recent hospitalizations who are at risk<br />

for re-hospitalization due to illness, safety or social factors will be offered<br />

Home Connections services. It is anticipated that clients with cancer,<br />

congestive heart failure, pulmonary disease, and chronic illness with<br />

multiple conditions, will benefit from a full range of palliative care services<br />

and support services to enable them to stay at home - safe, healthy and<br />

socially engaged.<br />

the Volunteer Office. The Volunteer<br />

Office works with the family in<br />

designing and incorporating the<br />

clothing into a blanket, stuffed<br />

animal, pillow, patchwork display/<br />

wall hanging, shadow box, framed<br />

cloth, etc. More than 15 of these<br />

memorials were made as gifts to<br />

families this past Christmas.<br />

A Call Center located within Life Transitions<br />

Center, and staffed by case managers, will<br />

be the hub from which all services will be<br />

coordinated. This Call Center will use stateof-the<br />

art technology to provide cost effective<br />

care management. Services provided by Home<br />

Connections will include a case manager to<br />

provide telemanagement and in-home care<br />

management planning; 24-hour on-call nurse<br />

and in-home assessments; 24-hour on-call<br />

home health aides; in-home palliative care<br />

physician assessment and visits; trained<br />

volunteers for telephone assurance, light housekeeping,<br />

meal preparation, shopping and pet care; connections to relatives<br />

and friends; emergency response team for ambulance, fire department,<br />

and utilities; transportation; lifeline; and outside services for legal,<br />

banking and home repair.<br />

While there are currently a variety of agencies that provide home-based<br />

support services, most costs for these services are not covered by Medicare<br />

or private insurance, especially if an older person has had a recent hospitalization.<br />

This places frail seniors who are on a fixed income at risk for<br />

avoiding outside help in their home. Home Connections partnership with<br />

Independent Health will help to address the need for quality, cost-effective<br />

support services to enable seniors to remain in their homes for as long<br />

as possible.<br />

Home Connections Palliative Care<br />

Management will directly provide or link all the social,<br />

health care, and quality of life issues necessary to make<br />

time at home the best option for Independent Health<br />

clients with serious illness.<br />

“The new Home Connections Palliative Care Management program<br />

will offer services that no other program does; combining health care<br />

service delivery with care management, volunteer visits, transportation<br />

and tele-management,” stated William E. Finn, president & CEO of<br />

After the pilot has proven successful, the program will be expanded to<br />

encompass all persons in Erie County who are at risk of hospitalization<br />

The Life Transitions Center and<br />

Essential Care for Children will be<br />

offering the 5th Annual Camp Blue<br />

Skies program, an overnight camp<br />

for any youth in Western New York,<br />

ages 7-16, who has experienced the<br />

death of a loved one three months<br />

to three years prior to the beginning<br />

of camp. Since Camp Blue Skies<br />

began in 2004, more than 145 youth<br />

have participated in the camp<br />

program. Camp Blue Skies offers<br />

therapeutic and recreational activities<br />

that allow children and teens to<br />

interact in age appropriate groups<br />

with professional grief counselors.<br />

Through small group activities and<br />

discussion, campers have the opportunity<br />

to share feelings, memories,<br />

and concerns about their loss.<br />

Campers connect with others who<br />

have experienced a similar loss and<br />

learn healthy ways to cope.<br />

Volunteers are needed as Camp<br />

Mentors to run activities, supervise<br />

campers and bunk in the cabins!<br />

It promises to be a meaningful<br />

experience for everyone involved.<br />

35- 40 volunteers are needed to help<br />

the weekend run smoothly. We are<br />

always looking for male and female<br />

volunteers, over the age of 18. We<br />

have a particular need to recruit<br />

male volunteers. Last year 29 of our<br />

46 campers were boys. It is critical<br />

to the success of the program to<br />

have positive male role models to<br />

St. John Baptist Church and <strong>Hospice</strong> <strong>Buffalo</strong> are pleased to announce that<br />

the St. John Baptist/<strong>Hospice</strong> <strong>Buffalo</strong> House, currently under construction<br />

on Maple Street in downtown <strong>Buffalo</strong>, will open in June of this year. This<br />

unique project is a collaborative effort between the church and <strong>Hospice</strong>.<br />

The St. John Baptist/<strong>Hospice</strong> <strong>Buffalo</strong> House will be the nation’s first<br />

residence for terminally ill patients developed jointly by a hospice program<br />

and a spiritual organization. This national model will provide hospice access<br />

and outreach services to residents of <strong>Buffalo</strong>’s East Side and throughout the<br />

broader community.<br />

In preparation for the opening residents, their family members, staff and<br />

The 13th Annual Artvoice Mardi Gras was held on February 5th in all<br />

three of <strong>Buffalo</strong>’s Entertainment Districts – Chippewa, Allentown and<br />

Elmwood. It began with a motorcade parade of over 30 floats through the area<br />

and ended with over 45 clubs participating in a New Orleans style celebration<br />

for everyone. Over $22,000 was raised for <strong>Hospice</strong> <strong>Buffalo</strong>. Thank you to<br />

Jamie Moses, Deborah Ellis, Rose Mattrey and the staff from ARTVOICE<br />

for coordinating this great event and to all the sponsors that helped make the<br />

night possible: Southern Comfort, Heineken, 103.3 The EDGE, Mix 104.1<br />

and 97 Rock. We would also like to thank all of the <strong>Hospice</strong> volunteers who<br />

volunteered their time that evening.<br />

Ann Schuldes, a current <strong>Hospice</strong> <strong>Buffalo</strong> patient. Mary Ann suffers from<br />

ALS, Lou Gehrig’s disease, and receives hospice services in her home. The<br />

letter describes her gratitude for the care she is currently receiving. With<br />

<strong>Hospice</strong> services she is able to have quality time at home with her husband.<br />

With Mother’s Day, Father’s Day and Memorial Day falling within a<br />

few weeks of each other, these occasions offer the perfect time to respond<br />

Mission Statement:<br />

The Center for <strong>Hospice</strong> & Palliative Care serves individuals<br />

with serious illness, and their families and caregivers through<br />

a continuum of care spanning the course of illness and<br />

bereavement. The Center’s specially-trained professionals,<br />

support staff and volunteers assist individuals to make choices<br />

about their care, support those who are grieving and educate<br />

the professional and lay communities about end-of-life issues.<br />

How To Access Our Services<br />

Affiliates of The Center for <strong>Hospice</strong> & Palliative Care are governed<br />

by the State of New York and most services are covered by a<br />

majority of insurance companies, including Medicare/Medicaid.<br />

Referring to the The Center for <strong>Hospice</strong> & Palliative Care is<br />

simple and convenient. Just call (716) 686-8000. An intake specialist<br />

will work with the patient, the family, and attending physician<br />

to initiate services and establish an individual plan of care.<br />

The Center for <strong>Hospice</strong> & Palliative Care consists of the<br />

following agencies:<br />

a centrally coordinated program of end-of-life<br />

education, research and practice.<br />

provides national education and training of<br />

bereavement and health care professionals.<br />

Includes the American Academy of Bereavement.<br />

provides medical care, as well as emotional and<br />

spiritual support to patients with advanced illness<br />

and their families.<br />

A 10-bed residence offering comfort and security<br />

for hospice patients in the final months or weeks<br />

of illness.<br />

An 8-bed residence in <strong>Buffalo</strong> offering comfort and<br />

security for hospice patients in the final months or<br />

weeks of illness. Opening June 2008.<br />

provides counseling and support groups for children,<br />

adults and families concerned with grief, death<br />

and coping with a serious illness.<br />

accepts, administers and invests all charitable funds<br />

on behalf of The Center for <strong>Hospice</strong> & Palliative<br />
